Adult Day Care
Brooklyn United Methodist Church Home Adult Day Health Care Program allows its participants to remain in their home within the community as long as possible in spite of physical or medical limitations.
Our Program has been carefully designed to provide a safe, stimulating and supportive environment in a lovely and pleasant state of the art facility. It is our promise to provide the best quality of health care services. We will work closely with your family/caregiver, community doctor and other service providers you have in the community.

Our experienced medical professional staff will develop a personalized care plan to meet the individual needs of each registrant in the program.
Schedule
We are open Monday to Friday, from 8:00 AM to 4:00 PM.
The number of days each person attends the Program will vary depending on his or her needs. (Flexible hours can be arranged.)
Applicants will be accepted into the program based upon medical needs & recommendation of a physician.
We are designed to meet the health care needs of chronically ill, frail elderly and disabled adults who require certain primary, preventive, diagnostic, therapeutic, rehabilitative or palliative service.
The services provided in the Adult Day Health Care Program are covered under the New York State Medicaid Program. Private rates are also available or contact your private insurance source and inquire for service authorization.
Trained drivers will provide door-to-door transportation in safe handicapped accessible vehicles. Free onsite parking.
